Our CPO 2011 volt came with its original charger but the little plastic hook is broken so it doesn't actually click in when charging. Would this be covered by our 12/12000 warrenty?

It IS probably covered. If your dealer resists replacing it... Call your Volt advisor at (877) 486-5846 and tell them you feel "unsatisfied" with your EVSE and they will help "adjust" your dealerships attitude until you get a new EVSE. That's what the Volt advisor program is all about... Helping YOU feel satisfied with your Volt.

Yes the EVSE would be covered but not by the Voltec warranty. The EVSE would be covered by the 36 month bumper to bumper warranty provided it has not been altered or damaged, i.e. cord cut or the J1772 connector broken, by the owner. If the dealer determines that the EVSE was damaged by lightning then it may not be covered.
