Can anyone tell me what form I need to send in to get the California rebate for the Bolt EV?

The dealership should be helping you with this, but the application is totally online for the most part. You do have to provide pdf scans of several documents to prove ownership and eligibility. There is an option to mail the docs in the old fashioned way if I recall.
